What choices do I have?
Your First Visit
Prepare for your first visit by completing and printing your New Patient Forms, which will ask about immunizations, allergies, medical history, and medications for each member of your family individually. Bring the completed forms with you for your visit.
Well Child Exam
As we get to know your child, we will schedule immunizations to prevent illness, learn about the child’s nutrition—is she a good eater, or does he refuse carrots?, and safety practices at school and home.
Well Teen Exam
That will include information about past illnesses, diet—does he like fast food or whole food?, health problems in the family—are there smokers in the house?, the teen’s school activities, interests, and friends. In addition, a first teen visit will include a physical exam to note growth and development in addition to a whole body check, including vision, hearing, and dental observations.
Men’s Health Care
We will discuss your lifestyle, including your diet, exercise, and weight, whether or not you smoke, if you use contraception, and your family medical history. You will have a physical examination.
Well Woman Exam
We will note your blood pressure, pulse rate, height, weight, BMI, do a urinalysis, a cholesterol blood test, a lipid profile blood test, and a diabetes blood test, and check your vision and hearing.
Well Family Care
When you want to see us together, we hope you will feel comfortable to call for an appointment and bring your cares to us.
